This form will submit your request for a banner to Tom Hartberg, who manages our banners. He will double-check the information to make sure that it will work as a banner for the website. He (or another member of the technology committee) will also follow-up with you if anything needs to be changed, or to help make some final decisions (like the background image, especially if you don’t have one).
